PROBLEM TO BE SOLVED: To provide a print instruction device, an image forming apparatus, an information processing apparatus, and a program that can reduce the amount of communication compared with a case where print data and biological information are separately transmitted.SOLUTION: A user terminal device 3 (print instruction device) comprises: acquisition means that acquires biological information measured about a user; print job creation means 302 that creates a print job (print instruction information) including the user's identification information (user ID) 36a, biological information 36d, a print condition 36e, and print data 36f; and print job transmission means 303 that transmits the created print job to a server device 4 (external device).SELECTED DRAWING: Figure 4
