PURPOSE: To maintain a horizontality on the top of a form even when the surfaces of a permanent form are rough and uneven.
CONSTITUTION: L-shaped members 1 and 2 are loosely connected to each lower end of a pair of net-shaped boards having reinforcing materials 5 and 6 at the upper edges. The L-shaped members are separated from each other at a specified spacing and installed side by side and fixed on a permanent form. The reinforcing materials 5 and 6 for each of the net-shaped boards 3 and 4 are temporality fixed at a predetermined home position. The connection between each of the net-shaped boards 3 and 4 and each of the L-shaped members are solidified. A spacer 15 is inserted outside from one of the opposed net-shaped board 3 and 4 into the other net-shaped board where reinforcing members 11 and 13 mounted at both ends which penetrate each net-shaped board of the spacer. A reinforcing member 9, which controls a specified width, is mounted between the reinforcing materials 5 and 6 for each of the net-shaped boards 3 and 4.
