PURPOSE: To prepare a routine form without specifying coordinates on paper and to improve system maintenance and management.

CONSTITUTION: This form preparing device is characteristically provided with a routine form preparing means 12 for preparing a routin form including a filling area for fill-in printing information individually recognized while checking a document output image on a display screen and adding which register information is stored in each filling area of the routin form, an information storing means 13 for storing register information to be inserted into the filling area to the routine form, a form interpreting means 14 for recognizing relation indicating which register/item in the means 13 corresponds to each filling area of the routine form, an information retrieving means 16 for extracting the corresponding register information from the means 13, a form preparing means 17 for preparing an information-inserted form by inserting the register information into the corresponding filling area, and an automatic form output means 19 for printing out the information-inserted form.
