A multi function peripheral 100 comprises: spread condition reception means 402 that displays in a selectable manner a spread full-page key; image condition reception means 407 that, when the spread full-page key is selected, displays in a selectable manner a predetermined number of top position keys for designating the top position of image data and a predetermined number of size keys for designating a paper size; spread image reading means 403 that, when full-page image data of a document in a spread state is read, causes image data storage means to store the full-page image data with the top position of the full-page image data being a top position corresponding to a selected top position key; and spread image forming means 406 that forms an image of the full-page image data stored in the image data storage means on a paper sheet with the size corresponding to a selected size key.
