A doctor selects the patient of a hospital ward to send an indication (S2). An indication input picture and a treatment item selection picture are displayed by patients. Any treatment item is selected out of the treatment item selection picture and indicated contents are inputted to the instruction input picture by the input device of the electronic medical record (S3). At such a time, a sentence example file is registered in advance and any indication sentence example is selected out of the sentence example file (S5). At a server computer, the indicated contents are recorded by sections of treatment items, stored in related recording files by patients and related recording files by sections and recorded on a hard disk or the like (S6).
