Abstract:
PROBLEM TO BE SOLVED: To obtain a delivery record by a method wherein one of superposed and bonded surfaces of a postcard main body sheet and respective affixing sheets, is made a confidential notice information listing surface, and on the opposite side from the superposed and bonded surface, of a delivery recording certificate, a receipt confirmation recording part and an identification code display part are provided.
SOLUTION: A delivery certificate 8 is superposed on the opposite surface side from the superposed and bonded surface between an affixing sheet 4 and a postcard main body sheet 3, and peelably bonded. In the meantime, a delivery slip copy 6 is superposed on the opposite surface side from the superposed and bonded surface between the delivery certificate 8 and the affixing sheet 4, and peelably bonded. Then, the superposed and bonded surfaces of the postcard main body sheet 3 and the affixing sheet 4 are respective made a confidential notice information listing surface. On the exposed surface side of the delivery certificate 8, a received date entry column 15, a receipt seal stamping column 16 which is a receipt confirmation recording part, an identification code display part 17a, and a peeling start and indicating mark 18 are provided.
