The system includes: a distributing means (a) for distributing to a patient terminal apparatus questions to obtain answers in response to the intelligibility of the patient, concerning an item which is at least one of the items, i.e., (1): an explanation for a disease in home health care, (2): an explanation for home health care, and (3): an explanation for a function recovery therapy (rehabilitation) concerning the disease; and a record holding means (b) for receiving the answers inputted in response to the questions by the patient from the patient terminal apparatus, and holding records so as to allow a doctor terminal apparatus and a linkage doctor terminal apparatus to read the records via a network.
